Long-term care referrals registry

The Oregon Department of Human Services (DHS) requires agents and other professionals who are compensated for providing long-term care referrals to register with DHS every two years. Referral agents must meet certain disclosure requirements, including providing consumers with information about their compensation practices, and whether they share a consumer’s personal information.  Connect with DHS

Overview

Placement and Referral Agents:

  • Must be registered by DHS
  • Must submit and pass a Criminal Background check
  • Carry General liability insurance
  • Are mandatory reporters
  • Must personally pre-view any facility they refer
  • Must provide a full disclosure statement to consumers
